About the Position

Job Title

Physical Therapist (PT) – Home Health

Location

Dickinson, ND

Schedule / Job Type

Contract | Days | Monday through Friday, 8:00 AM – 5:00 PM | No weekends or holidays

Key Responsibilities

• Conduct start of care assessments, recertifications, resumption of care, and regular patient visits in a home health setting
• Manage a typical caseload of 5–6 regular visits per day, or 3 additional visits on admission days
• Maintain a daily point target of 6.5 points per day
• Perform occasional safety evaluations or one-time teaching visits in hospice situations as needed
• Document patient care accurately and in compliance with home health standards, including OASIS
• Operate independently in the field while utilizing available clinical support resources

Required Qualifications

• Active North Dakota Physical Therapy License
• Current BLS certification
• Valid Driver's License and active Auto Insurance
• Ability to travel to patient homes within the service area

Preferred Experience / Skills

• Prior home health experience strongly preferred
• OASIS training and experience preferred
• Comfort working independently in the field with remote clinical support
